TB Risk Assessment Questionnaire
Very Important Information Regarding Assessment, versus TST and Blood Testing
We are currently performing TB Risk Assessment Questionnaire's free of charge by appointment
only for both employees and students, please call 619-660-4200 to schedule an appointment,
you can also contact your health care provider and have your health provider complete
the Risk Assessment Questionnaire and directly provide the Certificate of Completion
to HR.
Please note the TB Risk Questionnaire is a screening process for active TB, if TB
Skin Testing is recommended, this is a 48-72 hour process where a skin test is delivered
and you must return in 48-72 hours to have the test read.
TST's are free of charge for students.
New Employees in need of a TST are referred back to their Primary Care Provider or
a referral will be provided to a local health care organization that performs TST's
at a low cost.
TST's are done only on MONDAY and TUESDAY, so that you may return to have your skin
test read by an RN.
If you born out of the USA, an IGRA Blood Test is preferred over the TST and must
be done with your own health care provider.
For new employees: Please upload your Certificate of Completion for Tuberculosis Risk Assessment and/or
Examination into your Workday Onboarding task to the HR Technician with whom you are
working with.
